What Kind of Testing Does a Functional Medicine Migraine Specialist Use?
As a functional medicine practitioner, the labs I find most helpful in my clinic depend on the root cause and the unique individual's health history. In my experience, these labs have proved useful in solving for migraine's root cause:
Gut Health Labs — If gut health is suspected as migraine root cause, a deeper investigation might include a comprehensive stool analysis, which provides details on dysbiosis, absorption issues, inflammation, intestinal permeability, bacterial/fungal overgrowth, parasites, and more. We might also explore food sensitivity testing to gain insight on if the immune system might be treating food as a foreign invader.
Hormone Labs — To determine how hormonal imbalances might be affecting migraines, we might order a comprehensive hormone panel. This lab includes not only the sex hormones estrogens, testosterone, and progesterone, but also data on the stress hormone cortisol. We might also investigate thyroid health, like TSH, free T3, free T4.
Comprehensive Health Labs — Obtaining a solid view into the overall metabolic and mitochondrial profile of a patient is beneficial to migraine treatment. We look at traditional labs like CBC, CMP, fasting glucose, fasting insulin, HbA1c, lipid panel, vitamin D, magnesium, iron/ferritin, and B vitamins, especially B2 and B12, and a functional medicine lab like the organic acid test (OAT).
Toxic Burden Labs — If your health history points to possible toxic burdens, then we might order labs like mycotoxin panel (for mold toxicities) and also heavy metals.
Inflammation Labs — When our investigative process uncovers inflammation as a key migraine driver, we might explore labs like hsCRP, homocysteine, and histamine/DAO levels.
Keep in mind that while these labs provide helpful data, not every patient will need these labs. Most patients that I see typically only need one or two functional medicine labs in addition to traditional labs run by their doctor or healthcare provider. Often times the functional medicine lab kits are sent directly to your home, and you're able to collect the sample.
Does Functional Medicine Work on All Types of Migraines?
Functional medicine can be helpful for many types of migraines, including migraine with aura, without aura, hormonal/menstrual migraines, and chronic migraine, because it looks at the underlying drivers rather than the migraine subtype itself. Many of these root causes show up across different migraine presentations.
That said, functional medicine isn't a replacement for emergency or specialist care in every case. Migraines with certain red-flag features (sudden "worst headache of your life," neurological symptoms like vision loss or weakness, or a first migraine after age 50) need medical evaluation first to rule out other causes.
And some patients continue to benefit from working with both a neurologist and a functional medicine practitioner at the same time, rather than choosing one over the other.
The honest answer is that no single approach works for everyone. What functional medicine offers is a different set of questions — why are these migraines happening — which can uncover contributing factors that symptom-focused care doesn't typically investigate.

Do I need a referral from a neurologist?
No referral is required. Many patients come to DOTS Wellness after seeing neurologists or other physicians but still feeling stuck. My role is to look at migraines from a functional medicine perspective to explore possible root causes.
